The Care Coordinator (CC) is an integral component of the Plan’s Population Health Program and projects AHF’s core Values and Mission by providing cultural and linguistically sensitive supportive services to our special needs HIV/AIDS members. The CC works in tandem with multiple interdisciplinary members of the team, gathering relevant information and data to create a more fluid experience for the member.

Under the direct supervision of nursing leadership, the CC will facilitate scheduling transportation, appointments and outpatient procedures. Whether it’s helping a member attain needed records, change addresses or assist the nurse with completion of assessments, the CC is always reaching beyond to lessen the burden of a complicated health system to make it more accessible for the member.

The Care Coordinator utilizes our health plan’s services to improve member outcomes. To be successful, the CC must be willing to meet the member where they are, understand their needs and desires, and in a supportive fashion move the member along the continuum of care. By working to carry out the individualized plan of care developed by the nurse, the CC helps the member obtain increased autonomy and self-management of his or her healthcare.
