We are seeking a compassionate and dedicated Bilingual Korean/English Case Manager to join our team. In this role, you will work closely with clients to assess their needs, develop personalized service plans, and connect them with community resources. Your work will be instrumental in helping individuals navigate challenges and achieve their goals.

Key Responsibilities
Conduct comprehensive assessments to identify clients’ needs and strengths.

Develop and implement individualized service plans in collaboration with clients.

Provide case management services, including referrals to community resources such as housing, healthcare, and employment assistance.

Advocate on behalf of clients to ensure access to necessary services and support.

Monitor and document client progress, adjusting service plans as needed.

Facilitate workshops and educational sessions to empower clients with knowledge and skills.

Maintain accurate and confidential client records in compliance with organizational policies.

Collaborate with multidisciplinary teams to coordinate services and support for clients.

Qualifications
Bachelor’s degree in Social Work, Psychology, or a related field.

Fluency in both Korean and English, with excellent verbal and written communication skills.

At least one year of experience in case management or a related field.

Strong understanding of community resources and services available to the Korean-speaking population.

Ability to work independently and as part of a team in a fast-paced environment.

Excellent organizational and time-management skills.

Valid California driver’s license and reliable transportation
