Under supervision, you will perform a range of functions,
covering multiple pharmacy locations, units or other settings during a variety
of shifts, including but not limited to, float shifts. Your duties include the
preparation and dispensing of drugs in all inpatient settings; review and
verification of medication orders and patient medical records; optimizing safe
medication therapy; review of patient medication profiles; counseling patients
on appropriate drug use; interacting and/or rounding with other health
professionals to help provide safe and effective use of medications; provision
of advanced clinical services (e.g., pharmacokinetic dosing, therapeutic
interchange, IV/PO conversions, TPN and aminoglycoside monitoring, pharmacist
interventions, anticoagulation) to neonatal, pediatric and adult patients according to
departmental policy and procedures, with possible focus on pharmacy specialty
(i.e., oncology, pediatrics, geriatrics, ambulatory care); oversee dispensing,
return and waste of controlled substances on assigned
shift/unit. Initiate, innovate and complete clinical, operational and/or
management projects, in collaboration with Pharmacy Management. You will work
in assigned centralized and decentralized Pharmacy areas and patient care
units.

Schedule:
7 nights on followed by 7 nights off
Hourly
Salary Range: $86.32 – $107.31 (Union represented, step based salary
depending on years of experience)
Bargaining
Unit: HX-UPTE
Qualifications
We’re seeking a dedicated clinical professional with:
- Pharm D. degree and current CA Pharmacy license
- ASHP-accredited residency (preferred)
- 2 years inpatient pharmacy experience (preferred)
- Exceptional communication, interpersonal, organizational and follow-through skills.
- Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations.
- Ability to compound intravenous admixtures and other sterile products.
