When you join us as a Population Health Patient Navigator, you will be joining a dedicated team of professionals who deliver quality, value, and access in the 21st century and align all stakeholders- patients, providers, payors, and regulators to achieve the best clinical outcomes.
You Will:
- Serve as a liaison for patients identified through screening programs (e.g., lung, breast, or other population-based health initiatives) who require follow-up or additional care.
- Track and manage follow-up activities for patients with abnormal or concerning findings using internal systems and databases.
- Coordinate and schedule diagnostic follow-up, imaging, or consultations as needed.
- Communicate with referring physicians to support understanding and execution of follow-up recommendations.
- Maintain registries of at-risk patients and generate outreach reminders to ensure continuity of care.
- Assist patients in understanding their screening results and follow-up plans in a supportive and empathetic manner.
- Participate in the development, implementation, and evaluation of population health coordination and navigation programs.
- Develop and maintain a directory of internal and external clinical and support resources.
- Coordinate with JV partners to align workflows and data-sharing processes.
- Attend multidisciplinary team meetings, program trainings, and community outreach events.
- Ensure accurate documentation of all patient interactions and follow-up efforts.
- Report key performance indicators and program metrics to leadership.

To Ensure Success In This Role, You Must Have:
- Prior experience in patient navigation, care coordination, or a clinical support role preferred.
- Familiarity with population health concepts and screening program guidelines (e.g., Lung-RADS, BI-RADS, or similar) preferred.
- Experience with imaging systems (e.g., PACS/RIS) and EMRs highly desirable
- Excellent interpersonal, communication, and problem-solving skills
- Ability to work independently and manage multiple tasks simultaneously
- Proficiency in Microsoft Office (Word, Excel, Outlook).
- Strong attention to detail and ability to track and document data accurately.

Pay Rate: $30.00 per hour
