Job Summary: Key Job Responsibilities The Clinical Partner (CP), under the direction of a licensed care giver, and after education and demonstration of competency, performs a variety of direct patient care activities or tasks specific to the unit/needs of the patient population. The CP maintains competency skills including the ability to deliver care to special patient populations and patients with diverse cultural backgrounds.?? ?? Primary Duties and Responsibilities ??? Identifies physical, emotional and social needs of patients and reports findings to the RN. ??? Participates in the implementation of patient care and contributes to the plan under the supervision of the RN. ??? Provides for the safety and comfort of patients, families and significant others. Reduces the risk of health care acquired infection by following current CDC hand washing guidelines and CSMC isolation policy. ??? Safely uses restraints interventions under the direction of the charge nurse or primary RN. ??? Provides therapeutic communication, re-direction, reality orientation, effective motivation of patients to participate in activities, and adequate conduction of rounds and observations to maintain safety. Takes ownership for reporting problems and/or patient complaints. ??? Maintains patient stability by checking vital signs and weight, recording intake and output, dispensing/retrieving patient-care supplies, report abnormal vital signs to charge nurse or primary RN, as well as accurately detailing in electronic medical records. ??? Ensures environmental cleanliness in patient rooms and patient care areas. ??? Accompanies patients for any tests/procedures. ??? Assists patients with Activities of Daily Living (ADLs). ??? Assist patient mobilization and out of bed activities. ??? Identifies safety concerns in the age-related population and notifies licensed personnel of findings. ??? Participates in all initiatives that assist in reducing the risk of patient harm by following safety precautions.?? ?? Qualifications ?? Educational Requirements: High School Diploma/GED X Assoc. Degree/College Diploma Licenses: Certified Nurse Assistant (CNA) Certified Nurse Assistant (CNA) Certificate Upon Hire?? Basic Life Support (BLS) BCLS Certification Upon Hire?? Emergency Medical Technician – Emergency Medical Technician (EMT) Certification in lieu of CNA – Emergency Department Only.
