A Lead RN Medical Care Manager contributes to AHF by utilizing a strong understanding from a clinical perspective of the Ryan White Medical Case Management program and Standard of Care. The Lead RNMCM will work with our providers and Healthcare Center team to ensure that Care Management services are offered and executed in the manner to the Ryan White eligible population. The Lead RNMCM will provide training, clinical guidance to the team, and monitor services entered into the Care Management Information system. The Lead RNMCM will use the monitoring tool to ensure that your team is completing those measures that will be audited. By conducting 1:1 meetings with your team, you will understand the barriers and challenges and work to resolve them.
A Lead RN Medical Care Manager contributes to AHF by collaborating with their assigned supervisor for care management operations to grow their EMA/TGA. You will review the deliverables so they remain on target. Beyond the scope of the contract, your team will contribute to the measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services. Your oversight of the Care Management Program and participation in external support groups, consortiums, etc, adds to the AHF mission so that client/patient experience in Care Management is optimum.
Licenses and Certifications: Bachelor’s degree in Nursing (BSN) or License Practical Nurse (LPN) with at least 3 years of Case Management experience.
Ability to travel in your region up to 50% of the time.
