Your Role on the Team Position Summary: The Nurse Coordinator functions independently to coordinate care of patients. Responsible for planning, education and support of patient and family. Assures that the continuum of care process is complete, including intra-interdepartmental process to include: pre-admit, hospitalization, discharge and follow up in the OP. Maintains program data for analysis and RFPs. Works with Director and Medical Director / team for the development and growth of the program. Provides nursing services as needed. Other responsibilities as assigned. Require enhanced communication skills. Provides individualized nursing care and patient/family education that responds to the needs of patients and demonstrates an understanding of the range of treatment required by patients. Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments, etc. and assists as needed. Responds to the emotional needs of the patients by demonstrating sensitivity to the emotional state of patients and relating to patients in an appropriate manner. Refers to appropriate members of the health team as indicated. 20% Provides expert nursing care for patients with a variety of complex disease processes and a variety of treatment options. Reviews and interprets patient testing and communicates deviations from the norm with the physician/healthcare team. Serves as a contact point for patients, referring physicians and staff. Triages patients with urgent and or post-procedure, post-surgical care and a variety of medical needs and refers inquiries appropriately. Coordinates and facilitates the care of the patient in specialty areas. Provides leadership for collaborative patient care with emphasis on education and continuing of patient care for the entire period of care in the outpatient setting. Preforms nursing activities as ordered by the physician. 20% Provides direct and indirect patient care based on the nursing process. Demonstrates appropriate use of independent judgment and decision making. Provides education related to the specialty practices, diagnostics procedures, medication and other complex aspects of the plan of care. Incorporates the psycho-motor and age appropriate cognitive skills of the patient/family/significant other utilizing knowledge of the nursing process; patient assessment, planning, intervention and evaluation. Supports the faculty in clinic for patient care sessions. Administers and legibly documents the administration of immunizations and medications. Maintains current knowledge of pharmacology and competency in modes of administration. Maintains knowledge about the operation and care of biomedical equipment in the practice. 20% Oversees clinic and procedure schedules. Participates in business development/practice building strategies. Directs, educates and is a resource to authorization coordinators, medical assistants, schedulers and other registered nurses in other sub specialty areas. Manages the physicians’ practice that he/she supports. Participates in quality management activities related to the continuum of care by establishing and supporting standards, monitoring compliance, identifying patient care problems, documenting them appropriately and participation in resolution of identified problems. 20% Coordinates/facilities patient progression throughout the continuum of care. Assesses patient needs through observation of disease state, cultural and socioeconomic background and response to medical regimen and treatment. Uses knowledge of age-specific cognitive, physical, emotional and maturation process. Triages and responds or refers inquiries appropriately. Recognizes and reports specific physical and emotional conditions/needs/concerns of the patients to appropriate members of the health care team. Identifies and resolves delays and obstacles; offers and implements solutions to challenges/problems. Collaborates with physicians and other members of the clinical support team in planning for the needs of the patient. Required Qualifications: Must possess the skill, knowledge and ability essential to the successful performance of assigned duties Must demonstrate customer service skills appropriate to the job Minimum 6 months experience working as an RN in an Inpatient or Outpatient Oncology setting OR 1 Year clinical experience and recent completion of cancer basics class from Oncology Nursing Society within previous year Minimum 6 months experience working as a RN in an Oncology setting Minimum 2 years of clinical experience Minimum 1 year experience working in a clinical or inpatient setting providing RN care to leukemia patients Excellent written and verbal communication skills in English Demonstrated practice with and knowledge of chemotherapy agents Current CA RN license Basic Life Support certification required All external candidates must have a Bachelor of Science in Nursing Advanced knowledge of leukemia disease state and treatment Ability to maintain a work pace appropriate to the workload Preferred Qualifications: Oncology/Hematologic Malignancies patient population knowledge Oncology Nurse Certification Oncology / BMT patient population knowledge Knowledge of complications associated with medical and surgical oncology conditions Knowledge of complications associated with hematological oncology conditions Demonstrated knowledge of immunosuppressive therapies Demonstrated knowledge of immunosuppressive therapies and care of the post-transplant patient Bilingual skills to communicate effectively with patients and families Conditions of Employment: The University of California, Irvine (UCI) seeks to provide a safe and healthy environment for the entire UCI community.
