Make a difference in the lives of those experiencing homelessness in Glendale, CA! We’re seeking a compassionate and dedicated Outreach Case Manager to build supportive relationships, connect individuals to resources, and collaborate with the community to create pathways to stability.
Responsibilities:

Engage with unsheltered individuals and assess their needs.
Provide resources, transportation, and assistance in achieving case plan goals.
Partner with first responders, organizations, and community members to respond to calls for help.
Monitor client progress, maintain documentation, and provide crisis intervention.
Keys to hire-

Bachelor’s degree in human services or equivalent experience.
Social service experience with case management knowledge.
