Description
The Operation Mend Program is seeking a full time Patient Coordinator to provide clinic and
administrative support for the Operation Mend program. Major duties include:
- Coordination and scheduling of new and follow-up patient consultations,
appointments, labs, surgical procedures, and scheduling for Operation Mend. - Collaboration with UCLA physicians, social workers, and
clinical care teams to optimize the continuum of care for patients suffering psychological
and/or physical injuries. - Verify
new referral eligibility with the patient care teams and assist the patient coordinators
and other team members in creating concise and accurate patient files related
to their needs at UCLA Operation Mend. - Collect and manage all relevant clinical data (intake
forms, photographs, patient health history, labs, images, etc) prior to
consultations - Assist with coordination of video conferencing as needed
- Obtain
insurance pre-authorizations; coordinate additional appointments and insurance
authorizations with other specialists as needed. - Coordinate lab tests.
- Compile
and dispatch medical records to Operation Mend care team members in a timely
manner. - Be responsible for keeping up-to-date records, photos,
post-operative notes, and communicating directly with patients and caregivers
on a regular basis. - Work in coordination with colleagues at the other Warrior Care
Network sites to help assess programmatic components and assist in data
integration and process improvement.
Pay Range: $25.95 – $54.50
Qualifications
- Skill in
multi-tasking, analyzing information, problems, situations, practices, and
procedures to define the problem, identify relevant concerns or factors,
identify patterns and tendencies, formulate logical and objective conclusions,
and recognize alternatives and their implications - Recognizing and understanding patient priorities for scheduling and
follow up - Skill in
establishing and maintaining cooperative and collaborative working
relationships with administrative, clinical and academic personnel using
diplomacy and confidentiality - Knowledge of Care
Connect - Proficient in
Microsoft Office (Word, Excel, PowerPoint, and Outlook). - Working knowledge
of MCCS/PDM scheduling system - Knowledge of insurance
procedures, CPT and ICD-10 Coding - Working knowledge
of Salesforce - Demonstrated knowledge
of office procedures for specialty care as it relates to mental health,
including scheduling of appointments and procedures. - Extensive
experience in organizing workflow in order to accomplish established
objectives. - Excellent
verbal and written communication skills, with ability to edit documents using
proper grammar and syntax. - Ability to handle sensitive and difficult situations in a professional
and responsive manner. - Ability to develop priorities, meet deadlines despite interruptions, and
perform multiple complex assignments. - Ability to work independently on assignments with minimal supervision
