The Transplant Psychiatry Nurse Practitioner, in collaboration with the Multidisciplinary Team and supervising MD, will provide consultation and follow-up on patients referred for transplant evaluation and ongoing management in all inpatient and outpatient solid organ transplant programs. Essential Duties: Rounding on inpatients: providing orders, instructions, education in collaboration with the Supervising Physician(s). Management of the health care of the pre and post transplant patient population under the supervision of the attending physician. Performance of history and physical examination. Order diagnostic testing such as lab, Imaging, EKG and utilize results for formulating treatment plan. Review diagnostics studies and take appropriate action. Order non-operative therapeutic modalities. Prescribe medication as needed for symptom management. Complete medication reconciliation as required by CSMC policy. Administer medications as privileged and described as part of the plan of care. Provide patient/family education; provide patients and family with information on the plan of care. Record progress notes. Refer to multidisciplinary teams as appropriate. Assist the clinical team with triaging phone calls. Coordinating the discharging planning of patients and patient discharge education. Record discharge summaries. Qualifications Qualifications – External Education: MSN Degree required Graduate of an accredited Nurse Practitioner program required AGACNP preferred License/Certification: Current State of California Registered Nurse and Nurse Practitioner License required National certification as an NP required – Psychiatric Mental Health NP Current BLS certification from the American Heart Association or American Red Cross required DEA license required.
