The Semel Institute for Neuroscience and Human Behavior is seeking a compassionate and highly organized part-time Care Coordinator to support TIES for Families, a nationally recognized interdisciplinary program dedicated to improving outcomes for children with complex needs adopted from foster care. Working closely with the Director of Clinical Services and a multidisciplinary team, you’ll play a key role in coordinating care, supporting families, and ensure services are delivered seamlessly.

Primary responsibilities include:

* Coordinate services and referrals for children and families, connecting them with internal and community-based resources

* Serve as a point of contact for families, providing guidance, support, and advocacy

* Collaborate with clinicians, the Department of Children and Family Services (DCFS), and community parters to support care plans

* Schedule and organize appointments, case conferences, and team meetings

* Track case activity, maintain accurate documentation, and support billing processes

* Contribute to data collection, reporting, and program operations

* Support special projects and help improve systems that enhance care coordination

Please note, this is a limited position that may convert to career.

Hourly range: $30.36-$43.49

Qualifications

Bachelors degree in social work, psychology, public health or related field (highly preferred)Knowledge about providing client advocacy and liaison with community agenciesExperience in organizing and scheduling consultations and providing internal and external referrals when necessaryMinimum 3 years of experience in providing coordination of services and treatment for families involved in the foster care and option process (preferred)Ability to develop databases, analyze reports, utilize various communication tools and search engines using hardware and software programsAbility to communicate effectively with therapists and consultantsSkill in working as part of a team and collaborating with management, staff and supervisesExcellent interpersonal skills to interact effectively with professionals, staff, and clientsSkill in creative problem solving and being assertive but tactful in client case management in a fast paced work environmentAbility to handle multiple responsibilities and complicated issues successfully by maintaining control of the process and follow through on the relevant detailsSkill in formatting and writing grammatically correct chart notes, memos, letters, and reportsDemonstrated ability to handle confidential material with discretionMust be able to comply with departmental and program attendance standardsReliable source of transportation
