Incumbent is responsible for implementing, documenting and coordinating Healthy Steps program for high risk pediatric patients ages 0-3 and their families seen at the FHC in Santa Ana. Duties include assisting with integration of well child visits with multiple healthy steps screenings, conducting team based well child visits, data collection including caseload tracking, record keeping, and reporting, facilitating access to community resources and other referrals, maintaining community resource directory/database, interdisciplinary teamwork with all pediatric staff including LVN care coordinator. Ensures that all Healthy Steps program requirements are met and participates in all Healthy Steps trainings. Provides patient and family assessment, education, and counseling 25% Effectively collaborates with patient, family and pediatric healthcare team within the outpatient clinic setting. Enters assessment and case management notes on a daily basis for all patient contacts. Ensures accuracy of all documentation. Maintains accurate patient records of phone follow ups and response to case management. 25% Conducts and accurately documents outpatient psychosocial assessment, case management planning, community resource information, and all referrals. Advocates for participation of parents in planning for their children’s care. 25% Accurately coordinates and manages caseload of pediatric patients and families identified as high risk through Healthy Steps screenings, including proactive follow up and monitoring. Facilitates linkage with community resources to ensure patients are receiving needed services. Required Qualifications: Willingness and availability to work overtime as needed or requested Understanding of medical terminology Skills to understand and implement standards and regulations from various regulatory bodies. Skills to interpret and implement hospital, JCAHO, Health Department and Child Abuse Registry standards and regulations Proficient with MS Office (Word, Excel, PowerPoint, Outlook) Must possess the skill, knowledge and ability essential to the successful performance of assigned duties Must demonstrate customer service skills appropriate to the job Minimum one year mental health experience with pediatric population, experience and knowledge about early childhood growth and development, parent-child relational health, and family systems Minimum one year experience in health care setting Knowledge of and ability to work within a complex health care system Excellent organizational skills Current Clinical Social Work License issued by the Board of Behavioral Sciences Ability to maintain a work pace appropriate to the workload Ability to fluently speak, read, and write in English and Spanish Ability to establish and maintain effective working relationships across the Health System Preferred Qualifications: Knowledge of University and Medical Center organizations, policies, procedures and forms Demonstrated work experience with diverse cultures and low-income populations including homeless Licenses and Certifications: LCSW (Required) Conditions of Employment: The University of California, Irvine (UCI) seeks to provide a safe and healthy environment for the entire UCI community.
