Cedars-Sinai’s Emergency Department (ED), with 51 patient beds and 7-space Fast Track is a Level I Trauma Center, Level II Pediatric Trauma Center, Pediatric Critical Care Center, Level III Geriatric Care ED, STEMI Receiving Center, Stroke Receiving Center and Paramedic Base Station. The ED is open 24 hours a day, seven days a week available to all people in need. The ED Care Coordinator is a non-clinical team member skilled at engaging with patients and family during and after their ED visit, working with the ED???s clinical care team to enhance patient-centered care and outcomes following ED discharge. As an ED Care Coordinator you will function as a member of the care coordination team and perform the following tasks: Demonstrate Cedars-Sinai???s core values of respect, compassion and commitment to excellence in patient care through teamwork and collaboration. Patient advocacy through engagement, outreach, and providing linkage to resources. Educate and connect patient to medical home, facilitate post-discharge appointments, and medication or other at-home needs through social service providers. Assist patients and families with navigating community programs and services. Help patients access their health insurance, meet their social services needs, and follow-up on appointments. Work with area primary care and psychiatric clinics and providers to link patients with medical or psychiatric needs. Work with internal and external team members and organizations to ensure a coordinated outreach approach. Develop and maintain positive productive working relationships with health care providers and community-based services (e.g. mental health and substance abuse programs, community-based case management, and other support services). Maintain program records, reports and data. Follow up with patients to ensure they received the care and services recommended by the ED. Attend meetings as required and participate on committees as directed.
