What You will be doing in this Role: The social worker will work in a hospital-licensed s ocial Work FLOAT position, with rotations to ED, transplant, Out-Patient Clinics and Community Call. Hospital and/or medical clinic experience preferred t hat provides care for several patient populations?? They will have an independent clinic where patients are scheduled for social worker visits, to include clearance for surgery and/or counseling. Will see patients both in person and virtually. The social worker will also facilitate support groups/classes. Will collaborate both with our multidisciplinary clinic teams, as well as outpatient social work colleagues and inpatient social work/case management teams so as to ensure optimal care with inpatient to out patient transitions for complex cases.?? What are we looking for? Demonstrated ability to work with a multidisciplinary team. Risk assessment; Patient and family counseling; Qualifications Requirements: Education: Master of Social Work (MSW) required. License: Licensed Clinical Social Worker (LCSW) required.
