Nurse Navigator – Clinic – Surgery ENT – FT Days Updated: Jan 11, 2024 Location: Orange Job Type: Department: Clinic – Surgery ENT Job Description: UC Title: CASE MGR EX Position Number: 40321754 Reports to: Ambulatory Care Patient Supv 2 Working Title: Nurse Navigator Cost Center: Clinic – Surgery ENT (427263) Bargaining Unit: UPTE FLSA: Exempt Date Created: 1/04/2024 Job Code: 007870 Hours: 40 Shift: Not Applicable FTE: 1 Position Summary: The incumbent functions independently providing predominantly indirect and supportive patient and family care for head & neck cancer surgical patients that are treated at UC Irvine as part of the Otolaryngology Head & Neck Surgery, in conjunction with the Chao Comprehensive Cancer Center. Incumbent is accountable for the quality of nursing practice and functions as part of a multi-disciplinary team providing support for oncology patients and their families. Provides education as needed to the patients/family related to cancer care, diagnostic procedures, medication/chemotherapy and other complex aspects of the plan of care. The nurse incorporates the psycho-motor and age-appropriate cognitive skills of the patient/family/significant other utilizing knowledge of patient assessment, planning, intervention and evaluation. Assures that the continuum of care process is complete to include: pre-procedure counseling and needs assessment, peri-operative inpatient support, anticipated discharge needs, and follow up in the physician offices. Works closely with the outpatient head & neck oncology surgery RN Nurse Navigators and inpatient RN Case Managers, as well as with the medical oncology, radiation oncology, rehabilitation services, and social work teams. Provides coordination of care between ambulatory and inpatient transitions as needed and assists with other activities as assigned to help grow the practice and the referrals to UCI. Salary Range: Annual Rate Minimum $125,363.52 Midpoint $145,408.32 Maximum $165,432.24 Required Qualifications: Demonstrated clinical experience in skin and wound care Current CA RN license Ability to maintain a work pace appropriate to the workload Must demonstrate customer service skills appropriate to the job Excellent written and verbal communication skills in English All external candidates must have a Bachelor of Science in Nursing Minimum 6 months experience working as an RN in an Inpatient or Outpatient Oncology setting OR 1 Year clinical experience and recent completion of cancer basics class from Oncology Nursing Society within previous year Demonstrated practice and knowledge of post-surgical care Basic Life Support certification required Must possess the skill, knowledge and ability essential to the successful performance of assigned duties Preferred Qualifications: Bilingual skills to communicate effectively with patients and families Oncology/Hematologic Malignancies patient population knowledge Experience with DME equipment and supplies, as well as with skilled nursing facilities and home health agencies Conditions of Employment: The University of California, Irvine (UCI) seeks to provide a safe and healthy environment for the entire UCI community.
