Job Description In collaboration with the patient care team, the Patient Navigator works to remove the patient’s barriers to care by identifying critical resources, helping navigate through health care services and systems, and promoting patient health. The Patient Navigator operates with a high level of autonomy providing care coordination and workflow processing as well as communication to the patient at all levels throughout their care, at the direction of the care team. The Patient Navigator will leverage understanding of specific practice specialty, physician and clinic needs to ensure all applicable documentation, verifications, authorizations and equipment/supplies are attained to effectively serve the patient needs. Pro-actively facilitates the process and effective communication on all levels through tactful and professional interaction. Uses knowledge of the clinic to resolve complexed issues and identify ways to increase patient satisfaction and outcomes. Provides advance level of non-clinical and administrative support to the staff, physicians, and patients. The Patient Navigator will be the subject matter expert and provide training to others and conducting training session(s) on new processes/workflows. Serves as an ambassador, change agent, quality champion, and mentor/resource. Qualifications Education High School Diploma/GED – minimum Vocational/Technical Diploma Graduate of an accredited medical assistant program – preferred Bachelor’s Degree In a healthcare-related field – preferred Experience 3 years Support experience for a Medical office, ambulatory care facility or medical group practice – minimum 3 years Experience providing a high level of customer service in a fast paced environment, including handling confidential data with discretion. – minimum 3 years Expertise in insurance contracts and payor requirements. – preferred Licenses and Certifications Basic Life Support (BLS) by American Heart Association or American Red Cross Phlebotomy Certification CA – preferred CPT1 Certified Registered Medical Assistant – preferred
