Social Worker Case Management FT Rotate Updated: Apr 4, 2024 Location: Lakewood Job Type: Department: Case Management-LAK The Social Worker is responsible to facilitate care through resource coordination. This position helps patients achieve access to care and appropriate utilization of resources, balanced with the patients right to self-determination. The individual in this position has overall responsibility for and to assess the patient for discharge planning and transition needs, conduct complex psycho-social assessment and interventions to promote safe discharge preventing avoidable readmissions.
