Case Manager – Cardiac Cath Lab – P/T Days Updated: Mar 20, 2024 Location: Orange Job Type: Department: Cardiac Cath Lab UC Title: CASE MGR EX Position Number: 40936214 Reports to: Ambulatory Care Admin Mgr 1 Working Title: Case Manager Cost Center: Cardiac Cath Lab (427440) Bargaining Unit: UPTE FLSA: Exempt Date Created: 9/18/2023 Job Code: 007870 Hours: 24 Shift: Not Applicable FTE: 0.6 Position Summary: Incumbent collaborates with the attending and resident medical staff, as well as nursing, ancillary and technical staff to coordinate the delivery of high quality, individualized, cost-effective patient care. Coordinates resource utilization throughout the HealthSystem and evaluates services provided. Coordinates interdisciplinary referrals utilizing pre-established criteria for quality and appropriateness to the acute care or ambulatory setting. Performs the appropriate utilization review functions on the managed population utilizing approved Intensity of Service and Severity of Illness criteria. Supports the performance improvement process through comprehensive analysis of patterns and trends. Interacts with the patient/family. Utilizes nursing process, as appropriate, to set priorities, plan, organize and implement care that is directed toward self-care outcomes. Salary Range: Annual Rate Minimum $125,363.52 Midpoint $145,408.32 Maximum $165,432.24 Required Qualifications: Current CA RN license Ability to maintain a work pace appropriate to the workload Must demonstrate customer service skills appropriate to the job Excellent written and verbal communication skills in English All external candidates must have a Bachelor of Science in Nursing Minimum 2 years of clinical experience Basic Life Support Certification from AHA Minimum 1 year experience in Cardiac Care Setting Must possess the skill, knowledge and ability essential to the successful performance of assigned duties Preferred Qualifications: Bilingual in English/Spanish or English/Vietnamese (certain clinics may require this qualification) Previous experience in Cardiac ICU Conditions of Employment: The University of California, Irvine (UCI) seeks to provide a safe and healthy environment for the entire UCI community.
